The recipe is the opposite of the Enchanting Table – same materials, but placed in reverse order.

2. Click on the Disenchanting Table.
3. A UI will open showing available enchantments.
4. Choose the enchantment you want to extract.
5. 🎉 Receive the selected enchantment as an Enchanted Book.

Your original item will lose the selected enchantment, allowing you to reuse powerful enchants on better gear.
